5-Star Rehab Services
PHC is a 5-star community, accredited by the Centers for Medicare and Medicaid. Our therapists are committed to helping every patient achieve positive outcomes and return home as quickly as their recovery allows — with therapy available up to seven days a week.
Exceptional care begins with a detailed therapy evaluation. From there, our team designs a personal care plan and establishes clear, achievable goals — guided by your medical condition, your ability to participate regularly, and your physician's direction.
Aquatic Therapy
Our on-site Aquatic Therapy room features a 9-foot by 12-foot pool with an adjustable deck and underwater treadmill — giving patients of every level of function, including non-weight-bearing patients, full access. We're the only skilled nursing facility in the area to offer an on-site therapy pool.
Speech-Language Pathology
Our speech therapists focus on speech, voice, cognition, and swallowing, helping patients retrain and regain skills through exercises tailored to their needs — including patients affected by stroke, dementia, or the normal effects of aging.
Physical Therapy
Using specially designed equipment, our physical therapists help patients regain movement and strength — with support for gait training, bed mobility, wound care, and wheelchair positioning, so daily tasks become safer and easier.
Occupational Therapy
Occupational therapy helps patients regain the skills of daily living — eating, dressing, grooming, and housekeeping — so they can return to independent, productive lives, often following surgery, illness, injury, or age-related changes.
